  • 1. Personal & Dependent Information

  • This information is the foundation of your return.

  •  

  • * Valid Photo ID (for you and your spouse, if filing jointly)

  •  

  • * Social Security Cards (for you, your spouse, and all dependents)

  •  

  • * Birth Certificates (for all dependents)

  •  

  • * Proof of Dependent Residency: To claim a dependent, we must prove they lived with you. Please bring one of the following:

  •  

  •    * School, camp, or daycare records

  •  

  •    * Doctor, dentist, or hospital records

  •  

  •    * A lease agreement or landlord letter listing the dependent

  •  

  • * Childcare Provider Info: Name, address, and Tax ID Number (SSN or EIN)

  • * Alimony: If you paid alimony, we need your ex-spouse's Social Security Number.

  • 2. Income & Employment

  • Bring all records of money you earned. This includes:

  •    For Renters:

  •   Record of rent paid during the year

  •    * Student loan interest paid (Form 1098-E)
